This is the application form for SAPMA membership to be completed by Manufacturers and/or Suppliers who have passed a board resolution that this application be made, and that the Company bind itself, upon admission to such membership, to observe the Constitution and Code of Conduct of the Association.

In order to complete this application, you will need to have knowledge of your company's turnover for the past financial year, as well as digital copies of the following documents for upload:

• Company Profile • CCIPC COR14.3 Document • VAT Certificate

You will also need to provide the contact details of a company representative (at least one and no more than five).

We recommend that you compile the required information and documents before starting. You will, however, be able to save your progress during your application - you will be emailed a link to access your saved data.

This application may only be completed by a duly authorised signatory.
